TESTING AND ASSESSMENT SERVICES

Testing services are provided in office and in some cases via a combination of in office and remote meeting. All Assessments include a review of records, intake interview, behavioral observations, interviews with corroborative sources such as family members, teachers, others. and the formal assessments as listed  below.

The first step is to schedule a consultation interview of 1 hour. All testing requires $200 deposit at time of interview. In the consultation, the Psychologist will review the cost for services. Once determined, 50% is due at the time of the first assessment appoint. And the balance is due at the time of the last assessment appointment. (if assessments are completed in 1 day, full payment is due at time of rendered services that day). Services are non-refundable, non-transferable.

• In some situations, additional assessments may be recommended or required for your evaluation, which may result in an additional cost. Ex. Psychoeducational Testing points to probable ADHD.

• Fees vary by the battery of tests included in each evaluation, and range from $350- $1500. 

• Insurance may be billed out of network on your behalf, payable to you. Insurance does not always cover testing and if you plan to use insurance, you  may wish to call and ask for coverage of the following CPT codes: 90791 and 96130 and 96139.
